Registration General Information
All attendees must register for the conference. Attendees will be given a name badge which will be required for admittance to sessions.
When you submit your registration form, you are agreeing to all terms and conditions stated below.
The registration fee for the conference is $400 (U.S. dollars) if payment is received by October 11, 2006, and will cover coffee breaks, evening receptions, conference bag, and the organization of the conference
Registration receive after October 11 and/or payment on-site will be at the $475 rate.
Registration will not be processed without payment. The registration process will give participants the opportunity to purchase Reception tickets for spouse/companions. The fee will cover entry into the Welcome Reception (Oct. 30) and other evening receptions scheduled.
DEADLINE: Final deadline for registration is October 25, 2006. Form of payment must accompany the registration. Credit card numbers submitted with registration will be processed upon receipt.
REFUNDS: Requests for refunds will be received at the UH Conference Center by October 13, 2006, in writing. Fax requests to (808) 956-3364. There will be a $35.00 administrative charge on any refund/cancellation until October 13, 2006 . No refunds will be made thereafter. Refunds will be mailed. Please allow approximately three to five weeks for processing.
